Do the history yourself. Not at $250 an hour.

A large part of an ADHD assessment is gathering your history and scoring questionnaires. You can complete that at home, for free, at your own pace. Then we match you with licensed psychologists in your province who are ready to read it.

The usual way

You pay a psychologist to ask you about your childhood

—Two to three billable hours spent on history and intake before any testing begins.
—You try to recall school reports, jobs, and patterns on the spot, in an unfamiliar room.
—Questionnaires handed over on paper, then scored by hand later.
—You cold-call clinics to find who is taking adult ADHD referrals at all.
With our forms

You arrive with the history already done

—You complete the same history at home, free, over as many sittings as you need.
—Check old report cards and ask family, instead of guessing under time pressure.
—The questionnaire battery is scored automatically and arrives summarised.
—When you finish, you get psychologists licensed in your jurisdiction.

Your psychologist still conducts the clinical interview and makes the diagnosis. We are removing the paperwork hours from the bill, not the clinician.

How it works

Four steps. The first three are free and happen entirely at your own pace.

01

Request an assessment

A short form: your province, your age, and what prompted you to look into this. Two minutes.

02

Fill in your history

Childhood, school, work, health, family. Save and return as often as you like — nothing is lost between sittings.

03

Complete the questionnaires

The standard adult ADHD rating scales, plus screens for the conditions that commonly travel with it. Scored for you.

04

Get your psychologist list

Licensed psychologists in your province, with their fees and availability. You choose who to send your file to.

1Select your province

Where are you?

Why Canada only

Psychologists are licensed province by province, so a clinician in Ontario cannot assess a resident of Alberta. We only match within Canada, and your file is stored on Canadian servers under PIPEDA. If you live outside Canada, the history is still worth completing — but you will need to find your own assessor.

British Columbia

Our home province. Dr. Claire is registered here, and the network is deepest in Victoria, Vancouver, and the Lower Mainland, with virtual appointments available province-wide.

What the forms actually ask

The same context a psychologist covers in intake, in the order that makes it easiest to remember. You can stop and come back at any point.

Most people finish over two or three sittings across a week.

Childhood & school
Early behaviour, report cards, whether anyone raised concerns at the time.
Work & daily function
Job history, where things break down, what you have built to compensate.
Health & family history
Sleep, medication, mental health, and what runs in the family.
Rating scales
Standard adult ADHD measures plus screens for anxiety, mood, and related conditions.

Questions people ask first

Does this diagnose me?

No. Only a qualified clinician can diagnose ADHD. This prepares the file they need so the appointment is spent on clinical judgement, not data entry.

What does it cost?

Completing your history and questionnaires is free. The psychologist sets their own fee for the assessment; you see it before you choose them.

Who sees my answers?

Nobody, until you choose to send the file to a psychologist. Your data is stored in Canada and handled under PIPEDA.

Can I use my own psychologist?

Yes. The completed file is yours — take it to any clinician you like, in or out of our network.

How long does it take?

The forms take a few hours in total, spread over as long as you want. Your psychologist list appears as soon as you finish.

What if I already have a diagnosis?

Then skip this and go straight to coaching. Assessment is for people who still need an answer.

Start with the part you can do today

The history is the slow, expensive part of an assessment. Do it at your kitchen table instead, and arrive with it finished.
